Our Short Break service based in Grindon Area of Sunderland requires a dynamic and motivated Care Coordinator (Level 1) with the ability to work in challenging environments, to undertake a lead role in ensuring the care and welfare of our customers who require differing levels of support to meet a range of social and sometimes complex health needs. You are responsible to ensure the needs of young people and adults are met, give guidance and support to staff and work closely with the management team to ensure an outstanding service is delivered.
Our CQC Registered Short Break Services provide planned breaks to young people and adults who have a range of Learning and Physical Disabilities and Autism.
Your role will be on providing support to customers, promoting and developing their lives, health, and wellbeing. This may include:
Support with customer pre-assessments
Ensure person centre care plans are created and support with completing risk assessments in line with identified risks and needs of customers
Support with mentoring other staff that might be assigned to you by the Service Manager
Helping with preparation of meals & drinks
Support with domestic household tasks
Administer & record medication
Support with money management
Encouraging community trips and facilitating customers participation
Support with household & personal shopping
Support with meeting friends & family
Support with high standards of nutrition
Adhere to health & safety legislation
Cleanliness & intimate personal hygiene
Opportunities for employment or education
Provide emotional support
Active support techniques & positive behaviour support techniques to increase skill level & achieve set outcomes (training will be provided)
To support customer to plan holidays & support them if needed (additional payment)
