We are looking to recruit an enthusiastic, friendly and experienced clinician to join our team. You will need to possess recognised qualifications and experience to deliver autonomous care for our patients, including assessment, diagnosis, and treatment to deliver quality patient care relating to minor illness on a daily basis.
The successful applicant will be taking clinical responsibility for patients requesting an urgent appointment. As a team player, you will be working with and supported by GPs, PNs, and HCAs.
Main duties of the job
* To provide clinical support to the GPs and work closely with the wider clinical team to provide healthcare to our patients, always working within their professional boundaries and capabilities.
* To work within the practice to deliver care including assessment, diagnosis, and treatment in a holistic approach.
* Assess, diagnose, treat, refer, or signpost patients and service users with undifferentiated or undiagnosed conditions relating to minor illness/injury.
* Manage patients presenting with a range of acute and chronic medical conditions.
* Use your advanced clinical skills to educate our service users, promote self-help, and education.
* Support junior staff and the nursing team working within the practice.
* Demonstrate safe clinical decision-making and expert care for patients and work collaboratively with the multi-disciplinary team to meet the needs of our patients.
* Process and interpret pathology and other test results as required.
* Maintain accurate clinical record keeping in accordance with NMC.
* Support the teams in QOF/IIF contributing to practice targets.
* Participate in PLT sessions and daily clinical huddles for advice and support.
* Always follow Safeguarding policies and support staff.
About us
The practice has a list size of approximately 5,500 patients. Our practice team consists of 2 GP partners, 3 salaried GPs, and a well-established nursing team, with an experienced admin and management team. All the departments and teams work together to deliver a safe and excellent service to our patients. We are set in the town of Poole, very close to Poole Park and the waterfront. We are part of Poole Central PCN and served by University Hospitals Dorset, including Poole, Bournemouth, and Dorchester.
Job responsibilities
Assess, diagnose, plan, implement, and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis. Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly. Prioritise work when working on duty triage and manage time accordingly. Expectations to always deal with medical emergencies within the practice. Support the nursing team when needed by reviewing cases and advising on treatment. Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long-term condition. Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan. Review and follow up patients as required to facilitate a good standard of care. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent, or emergency situations, including initiation of effective emergency care. Recognise, assess, and treat patients suffering from mental health conditions, following the referral process if necessary. Work within own scope of expertise and ask for help and advice when needed. Attend daily clinical huddles for reflection and feedback. Complete regular audits. Teach and mentor learners where appropriate. Be involved in team/practice learning sessions.
