Role Summary To have 24-hour responsibility for the provision of specialist palliative nursing services to meet the individual needs of patients on the Inpatient Unit. Support the Director of Care & Operational Services to provide effective clinical leadership to the immediate clinical team members To be accountable for defining and ensuring that agreed professional standards are reflected in practice within the nursing team. To be accountable for the overall management of the nursing team, recruitment, deployment, and supervision of staff ensuring the co-ordination of all services provided to the patients. Optimise the cost-effective use of all resources. To identify the core and extended competencies required by clinical staff and assist the Education & Training Lead to develop a learning evidenced based programme to support staff to achieve them. Demonstrate Clinical Leadership and expertise in Palliative Care whilst disseminating this Specialist knowledge to Clinical Staff Main Duties and Responsibilities. To provide clinical leadership and management support to the Inpatient Unit(IPU) and Day Hospice teams ensuring that the highest standards of holistic nursing care required by the Regulating Bodies, Commissioners of Service, and Board of Trustees are provided through supervision and operational management of the unit. Monitor and maintain colleagues clinical competencies within Hospice and regulatory guidelines Develop and implement a model of nursing care which ensures each patient has an individual care plan which includes assessment, planning, implementation, and evaluation of care. Work with the multi-disciplinary team to provide a high standard of specialist palliative care to patients and their carers within the hospice. To participate in multi-disciplinary meetings. To attend clinical meetings relevant to role as required. Act as a specialist resource for patients, carers, and non-specialist groups of staff, promoting patient carer involvement. Maintain accurate, up-to-date staff records. Manage Staff and other resources effectively, support the DOCOS and Registered Manager to maintain continuity of care, allocation of annual leave, provision of rota, monitoring sickness Play a lead role in the recruitment, selection, and induction of Clinical Staff Report, and where appropriate investigate, clinical incidents, formulating a clear account of the incident and agree outcomes in consultation with PSS, QA, DOCOS and Registered Manager Ensure staff are made aware of any new clinical policies and procedures arising as a result of audit and other monitoring processes and that any identified associated training is provided To maintain specialist expertise underpinned by theory and to maintain clinical contact so to act as a role model to nursing staff. Develop clinical protocols to facilitate practice developments, benchmarking where possible, to ensure that staff adhere to bestevidence-basedpractice and makes optimum use of resources to achieve the highest standards of care and service. Maintain a safe environment for patients, visitors, staff, and volunteers, ensuring that staff and volunteers are aware of their responsibility in relation to health and safety, and cleanliness. Take an active role in quality assurance initiatives, including the setting and auditing of standards and implementing resulting action plans. Organise learning and development resources using agreed methods and frameworks and contribute to the delivery of in house training. Support and maintain link nurse programme with involvement of Education Lead Coordinate career conversations for clinical team allowing opportunities for growth & development of role. Collect and collate data and keep knowledge and information systems up to date using set procedures. Be aware of organisational strategic direction and future issues pertaining to the provision of specialist palliative care and supportive care. Have involvement in the training and support of students on placement and where applicable provide mentorship Ensure the Hospice complies with all the training requirements outlines in the Service Level Agreement for the provision of blood and other associated products for transfusion To assess and review patient dependency, adjusting staff requirements and advising the Registered Manager of any changing needs to meet the CQC requirements and reduce risk to patient safety Ensure staff are made aware of any new clinical policies and procedures arising as a result of audit and other monitoring processes and that any identified associated training is provided Policies and Procedures Understand and comply with the policies and practices of Halton Haven Hospice. Undertaking of policy development and review in line with set schedule of work Attend statutory training in accordance with Halton Haven Hospice requirements. Education and Training Undertake any training that may be necessary to completely carry out the post and to attend all mandatory training. Actively participate in the personal development and education of self and team. Facilitate an environment, which will be conclusive to learning. Undergo career conversations in line with Hospice policy and procedure. Maintain own personal and professional development. Communication and Working Relationships To carry out all duties involving patients and their families in a courteous and respectful manner. To report all mishaps/accidents/incidents, infection control concerns or Health and Safety issues, in accordance with Hospice policy and procedure, to the person in charge, as soon as practicable. To report any equipment service needs, shortages and/or electrical faults to the Facilities Manager (or maintenance staff) as and when they occur. To respect the confidential nature of all patient information. Ensure a warm welcome to all visitors to the Hospice demonstrating tact, empathy and awareness of social and cultural differences when communicating with patients, relatives, visitors and other professionals. Maintain all written and electronic patient records in line with Hospice Policy and Procedure, ensuring confidentiality an compliance with Care Quality Commission Regulations and the Data Protection Act. Ensure all communication is recorded and disseminated as appropriate. General To act responsibly at all times ensuring that any actions do not bring the Hospice into disrepute. To contribute to the team working and efficient functioning of the Hospice. To ensure compliance with the hospice policies and procedures and have regard for the employees obligations under the Health and Safety at Work act 1974. To attend Fire Safety Training as required and demonstrate awareness of the correct procedure to be undertaken in the event of a fire. Work within the Hospice Policies and Procedures at all times. Maintain security required for safety of patients, staff and the general public. Follow Hospice procedures for managing positive Infection Control and Health and Safety practices. Ensure the effective use of Hospice resources. Attend and participate in meetings and events as may be reasonably required by the Hospice. Ensure Infection Control is maintained in line with requirements specified in Hospice Policies and Procedures.
