We are looking for Registered Nurses who would like to work as part of our inpatient team providing patient-centred care for people living with HIV.
Responsibilities include:
1. Referral management
2. Assessing and planning care
3. Discharge planning
4. Medication management
5. Symptom management
6. Palliative and end of life care

Main duties of the job
Work with the nursing team providing individualised care in accordance with the organisation's Policies and Procedures.
1. Contribute to daily activities including referrals, admissions, medication management, care planning, and discharge management.
2. Plan own workload and support colleagues as needed.
3. Responsible for own documentation ensuring collaboration between members of the multi-disciplinary team in delivering patient care.
4. Ensure effective communication within the multi-disciplinary teams both within the organisation and other outside agencies.
5. Act as the patients' advocate respecting patient privacy and dignity.

Our specialist ten-bed HIV community hospital is set just outside of Brighton and Hove. We pride ourselves on delivering person-centred holistic care, creating therapeutic alliances, promoting health, and self-management with all our patients. We manage a wide range of clinical and mental health care including acute stepdown, continuing care, crisis prevention, drug and alcohol detox programme, high dependency care, medication management, mental health support, and palliative and end of life care. We work in partnership with HIV services across the south coast including those from the voluntary sector.
Job responsibilities
The postholder will be responsible for the management and care of designated inpatients on a shift-by-shift basis, ensuring best practice and safe care delivery.
The post holder will:
1. Be responsible for processing referrals, assessment, development, implementation, and evaluation of high standard programmes of care for all patients.
2. Work within The NMC Code 2015 of professional standards of practice and behaviour for nurses and midwives.
3. Use specialist skills, advanced practice, knowledge, professional judgement, and competence in line with organisational policies.
4. Support the professional development and training of students and colleagues.

Qualifications
* NMC Registered Adult Nurse
* NMC Registered Mental Health Nurse
Experience
* At least 1 year post qualification experience.
* Experience of working in a team.
* Able to demonstrate relevant experience of working with people from diverse backgrounds.
* Experience of working with people living with HIV (PLWHIV) or other long-term conditions.
* Understanding of the current challenges for people PLWHIV.
* Experience of working with people living with mental ill-health.
* Experience of working with people who use alcohol and non-prescribed medications.
