Family Coordinator
Overall Purpose:
As a Family Coordinator, your primary role is to provide direct support and targeted interventions to families with children aged 0-19 years (or up to 25 for those with Special Educational Needs and Disabilities - SEND) who face complex and interrelated challenges. You will work collaboratively with families, using a strength-based approach to co-develop assessments and family support plans. In partnership with other professionals, you will identify effective ways to support children and families, ensuring the appropriate sharing of information. The aim is to enhance family resilience, drive positive change, and minimize the need for more intensive interventions.
Key Responsibilities
1. Conduct direct work with children and families, carrying out comprehensive early help assessments to determine their level of need and risk, and developing outcome-focused family support plans.
2. Lead and coordinate the Team Around the Family (TAF) process, ensuring a well-organized support package is in place.
3. Identify safeguarding and risk concerns in line with established policies and procedures.
4. Ensure the voices of children and parents are central to all aspects of the work.
5. Engage families creatively, fostering strong professional relationships and encouraging participation in the available support services.
6. Utilize trauma-informed practices to build on family strengths and achieve positive outcomes.
7. Maintain accurate case records and prepare reports for multi-agency meetings, case conferences, and legal proceedings as required.
8. Reflect on and evaluate personal practice and service delivery to promote best practices and enhance outcomes for families.
9. Uphold confidentiality standards and ensure appropriate information sharing in accordance with policies.
10. Work flexibly across the East Riding of Yorkshire to meet service demands, including early mornings, evenings, and occasional weekends in family homes.
11. Demonstrate English language fluency as required under Section 77-84 of the Immigration Act 2016.
12. Carry out additional duties as required by the Director of Childrens Services or their delegated officer.

Ensure personal health and safety by adhering to all relevant legislation and council policies, raising concerns with managers when necessary, and taking appropriate action.
Data Protection And Information Security
Comply with the Information Security Acceptable Use Policy and Data Protection Policy. Safeguard council information assets from unauthorized access, disclosure, modification, destruction, or misuse. Report any security incidents or potential breaches.
