An exciting opportunity has arisen in the Devon Onward Care team and the South Hams And West Devon Community Social Care Team. You can be at the forefront of supporting people to return to the community with complex care needs.
You will be responsible for facilitating complex discharges from Derriford Hospital and reviewing the service users' care needs within the South Hams and West Devon communities once they have been discharged into their community care setting.
We have adopted the Discharge to Assess Model which is facilitating complex discharges from the hospital. You will be based at Derriford Hospital and in the community and will collaborate with our hospital and community-based colleagues within a whole integrated system.
The role provides an excellent opportunity for anyone looking to progress their social care career. You will be working alongside a health and social care team that comprises Discharge Nurses, Senior Social Workers, and Social Care Assessors from the South Hams and West Devon Community Health and Social Care Team.
This role would suit an individual looking for a fast-paced and varied role that will include the completion of strengths-based social care assessments, coordination of care, placements, and ongoing social care support following discharge from the hospital.
The nature of this work requires you to be highly organized with strong recording skills. You will receive excellent support in a busy and friendly team. You should be an effective communicator both verbally and in written formats, able to prioritize workload, manage conflict effectively, be flexible and enthusiastic, have good time management skills, and be able to work under pressure. You need to be IT literate and willing to develop your skills. You will be committed to equality and diversity, good customer service, and achieving positive outcomes for vulnerable people.
You will be fully supported and given training where needed and be fully supported by a friendly, welcoming team.
