Job summary Do you have experience in managing one or more long-term condition within a Primary Care environment? Or are you an experienced Practice Nurse who has expanded their skills and has experience in managing long term conditions? We are recruiting an enthusiastic Chronic Disease Specialist Nurse to work across Lakeside Surgery, Corby and Brigstock. As a Specialist Nurse, you will provide and maintain a high standard of chronic disease management, which includes diagnosis, monitoring and development of individual management plans and agreeing these as appropriate with the patient and other health professionals. Main duties of the job The post holder will work collaboratively within the multi-disciplinary general practice team, to meet the needs of patients, supporting the delivery of policy and procedures. The post holder will provide specialist assessments and reviews of health care needs, the optimisation of care where co-morbidity exists, maximising wellbeing, self empowerment, and quality of life.
