We have an exciting opportunity for a driven individual to join our Porth Preseli Team. We are looking for a dedicated individual with a passion for providing patient centred care to the population of Pembrokeshire.
The overall aim of Porth Preseli is to provide a high quality, comprehensive service for adults who require support in the community through the provision of a single point of communication, coordination, and triage for all routine, intermediate, urgent and crisis referrals/requests.
Main duties of the job
You will:
1. work as part of an MDT to make clinical decisions within Porth Preseli, Hospital@home response to ensure the correct response to a new referral as part of admission avoidance.
2. co-ordinate the daily MDT and highlight any significant changes or events which may indicate further action.
3. ensure that the care given meets the standards and policies set by Hywel Dda.
4. ensure that there is effective communication with other departments and services (including Accident and Emergency, Ambulance and out of hours).
5. work with all members of the Porth Preseli H@H team and other colleagues within the health board acute and community services and local authority.
6. liaise with hospital colleagues in the event of a patient from H@H being admitted to hospital or attending A&E.
7. ensure that all discharges from the caseload are discussed and agreed.
8. be flexible to meet changing demands at short notice.

1. Proven and relevant experience at Band 6 within a community setting.
