Job summary We have a number of exciting opportunities available within the South Tyneside All Age Continuing Healthcare Team employed by North East and North Cumbria Integrated Care Board. The posts are open to nurses with RGN, RNLD or RMN registrations. Reporting to the All Age Continuing Care Clinical Lead Nurses, you will be part of a dedicated team working within the health and social care system. Main duties of the job You will be responsible for the specialist Continuing Healthcare assessments, reviews and case management of the health needs of individuals either in their own homes, residential, specialist accommodation or nursing accommodation and act as a point of contact, referral and advice, completing regular reviews for health funded individuals and ensuring compliance with NHS Continuing Healthcare and NHS Funded Care National Framework Guidance (2022).
