Family Support Practitioner
Salary: £25,950 per annum
Location: Caerphilly
Contract/Hours: Fixed term contract until 31 June 2025 - 37 hours per week

A bit about the role
Action for Children's Gwent Family Intervention Team is a targeted early intervention therapeutic service for children, young people and their families aged 3-16 delivering across Caerphilly. Our work is underpinned by psychological therapies including attachment, systemic psychotherapy, and behavioural approaches.
Our aim is to reduce the development of more entrenched mental health issues by delivering high quality therapeutic family support which can involve one to one support with parent and child, as well as whole family sessions. The service offers needs-led interventions that are focused on each family being regarded as unique using formulation and curiosity approaches to help build an understanding of the context that the family live in. We aim to empower families to help support relationships, improve communication patterns, build on resilience, and explore healthy ways to communicate feelings and emotions.
How you'll help to create brighter futures
As a Family Support Practitioner you'll:
* Working with a range of professionals to ensure the children and young people you support develop their full potential.
* Working with a Practice Team Leader, Clinical Psychologist to explore the needs of children and young people where emerging social, emotional, behavioural and emerging mental health issues have been identified.
* Delivering a time limited intervention up to 12 weeks in collaboration with the child or young person and their family.
* Liaising closely with your Practice Team Leader, colleagues and our partners to ensure compliance an excellence.
Let's talk about you
We're looking for a Family Support Practitioner who has:
* Strong work experience of working with children, young people and their families providing one to one and family work in the community.
* Ideally you'll have experience of delivering targeted early interventions for children and young people.
* A proven record of working collaboratively with schools, social services and health partners and responding to safeguarding concerns.
* Experience in managing and holding a caseload of 10 minimum including managing risk and safeguarding.
* Experience of attending and inputting to Team Around the Family (TAF) meetings to meet the needs of vulnerable children and families.
* Experience in delivering time-limited interventions including one to one support and group work delivery and facilitation
* An ability to work on your own initiative but as part of a wider team.
