As part of the Incident and Learning team, the post holder is responsible for the management and delivery of the Trust's process for managing and reviewing incidents and ensuring that the Trust is compliant with national and regional targets relating to this. The post holder will ensure that all incident systems are maintained to the highest standard. The post holder will support colleagues in clinical and operational services in the effective and timely review of all incidents aligned with the patient safety incident response framework (PSIRF), lessons learned and the development and completion of SMART action plans. They will also support colleagues to raise awareness of patient safety concerns and incidents involving colleagues and visitors. The post holder will support those undertaking patient safety incident investigations (PSII'S) to assist with the process. The post-holder will play a key role in trust wide learning from incidents and support the patient safety board operational group, the safety assurance group. They will also support the Service Groups with understanding the requirements set out for assurance and to collaborate on developments in these areas as they change. To ensure that themes, trends, and patterns arising from incidents and near misses are understood and addressed.
Main duties of the job
The Incident and Learning Lead will have a visible presence in the Trust representing the Governance Support Team and be responsible for leading effective, timely and proportionate responses to patient, colleague and visitor safety incidents. The post holder will work autonomously within a team of Incident and Learning Leads with support from the Head of Patient Safety and Learning when required. The post holder will also ensure that Duty of Candour requirements are met.
The Incident and Learning Lead will support the Head of Patient Safety and Learning to provide Trust-wide support, ensuring high quality reviews and effective learning and improvement as part of the Trust's agreed Patient Safety Incident Response Plan (PSIRP) as required within the Patient Safety Incident Response Framework (PSIRF).
The post holder will regularly communicate highly complex, sensitive and distressing information relating to patient safety incidents compassionately and effectively to a range of stakeholders including members of the Trust, patients and families.
The post holder will also ensure all Trust staff have the specialist knowledge and skills to improve patient safety and learning through the delivery of training, teaching and leading by example in the facilitation of incident reviews. The post-holder will also act as an expert resource providing advice, support, and mentorship to patient safety incident (PSI) investigating officers, and by exception, undertake PSII's and reviews.
