Main duties of the job
As a Community Nurse - Band 5, you will play a pivotal role within the Community Nursing Team, supporting patients with diverse health needs in their own homes. Your focus will be on providing holistic, high-quality, evidence-based care while promoting patient independence and recovery.
You will:
1. Conduct first visits, address immediate care needs and report to the Team Leader for further discussion or assessment.
2. Provide care according to the available evidence of best practice and the support of others in the development of evidence-based practice.
3. Actively promote self-care for patients and integrate Health Promotion into care.
4. Develop and participate in planned teaching programmes for patients.

Job responsibilities

* NMC current pin/or about to qualify.
* Full UK driving licence, and either have access to a vehicle which can be used for work purposes or are able to travel as required to meet the needs of the service. *Please note not all office locations or client care bases may be reached by public transport.
* A passion for community nursing that matches our passion to provide excellent healthcare for patients within their own homes.
* Willing to undertake training to fulfil the requirements of this role.
