The Health Care Support Worker role will function as part of the team within the Discharge Lounge to provide a seamless service & high-quality care to patients under the direct supervision of a Registered Nurse. The HCSW is expected to undertake care to meet the needs of the patients and relatives under the supervision of a qualified nurse, working within the Trust's policies and procedures and the philosophy of the Discharge Lounge. Working collaboratively with wards and ED in the care of patients being discharged from the hospital, and providing a safe and comfortable environment before discharge. It is expected that the Health Care Support Worker will continue to develop their own capabilities and knowledge of nursing tasks in line with the Trust's career pathway.
Main Duties of the Job
1. Collecting patients from wards who have been identified ready for discharge lounge, ensuring all patient property, medications, and discharge documentation are with the patient, if available.

The post is advertised as a band 3 role subject to the following:
* Successfully completing the care certification within the six-month probationary period of employment, during which you will be paid Band 2.
or
* Providing a copy of your completed care certificate which has been reviewed during the recruitment process and determined as meeting the accepted standard.
Upon successful completion (or accepted evidence) of the Care Certification and completion of the probationary period of employment, you will be promoted to NHS AFC Band 3.
Failure to complete the Care Certification within this timeframe may result in termination of employment.

Education and Qualifications
* Good general education with Maths and English GCSE or equivalent.
* Good written and spoken English.
* Experience of using own initiative to resolve problems.
Experience
* Previous experience in a healthcare setting.
* Experience or understanding of working within a team environment.
* Previous experience of working in a similar role.
Skills and Knowledge
* Extensive previous experience in healthcare environment.
* High standard of IT skills with the ability to input and retrieve information with accurate data entry.
