At Derian House Children's Hospice in Chorley, Lancashire, we are looking for an enthusiastic GP who can support our clinical strategy to grow services and help more children and young people with life-limiting conditions throughout the North West.
You will be joining us as a self-employed GP advisor, and will work alongside our other GPs on rota in supporting our day-to-day medical model and providing on-call cover.
At the hospice, we care for more than 400 babies, children, and young people and their families, helping them to make the most of every moment. Do you have the care, experience and skills to make a difference?
1. A daily contact or in-person visit to the hospice during your week on call, Monday to Saturday, to assess the needs of individual children and young people, and to provide appropriate advice and treatment. This includes symptom management, death certificates, diagnosis and treatment of acute conditions.
2. To be available out-of-hours for the week you are on call to provide advice, emergency assessment and treatment of a child's or a young person's condition when a deterioration has occurred, symptom management, or where changes are occurring in the end of life stage, and certification of death when required.
3. The prescription of medications, when required, following Derian House Standard Operating Procedures, taking cognisance of approved formularies.
4. Involvement of the child and their family, where appropriate, in any decisions about proposed changes to planned care.
5. Documentation of all consultations in a child's/young person's notes, in line with the GMC code of practice.
6. Liaise appropriately with care staff at the hospice, with paediatricians and family GPs to ensure discharge information is communicated in a timely manner when changes have been made, and to report to the Chief Nurse.
7. Attend a monthly one-hour meeting of all GPs, which takes place at least 9 times per year.
8. To undertake continuous professional development appropriate to this post, so as to evidence-base 16 hours per year of this, paid for by Derian House.
