The Intermediate Care at Home Team provides short-term re-ablement to people in the community who need to regain their confidence and skills following discharge from hospital or after a fall or unforeseen health issues. Our service operates 7 days a week, 365 days a year, working in liaison with other healthcare professionals to support those in most need, helping them return to living as independently as possible.

Main duties of the job
Job title: Integrated Support Worker
Full and part-time positions are available on a rota pattern between the hours of 7 am and 10 pm, including weekends and bank holidays.
This is a permanent position.
The Integrated Support Worker is a key role in the delivery of our quality service. We provide an extensive induction and training programme to ensure you feel empowered to deliver an effective, safe, and enabling service to our service users.
You will work in partnership with physiotherapists, occupational therapists, nurses, and care managers to enable service users to achieve their best quality of life. You will support service users identified through an assessment process and act upon initial referral information, responding flexibly to the patient/clients' presenting needs. You will also be required to administer medication in accordance with policy.

You will need an NVQ 2/QCF in Health and Social Care or an equivalent qualification or be willing to work towards this, as well as a standard level of literacy and numeracy skills and a basic level of computer skills, including Microsoft Word.
