Job summary Portland Medical Centre, as part of Bourne Health are looking for a proactive and enthusiastic Pharmacist with prescribing qualifications, preferably with hospital pharmacy experience, to join our Practice to work closely with our nurses and GPs. The appointed Pharmacist would be part of a wider pharmacy team working with our associate practices within a Hub model. Main duties of the job As the role will develop the successful candidate will take responsibility for areas of Chronic Disease Management and undertake Clinical Medication Reviews, proactively manage patients with complex poly-pharmacy needs.
