Jacksonville, FL 32257Works in collaboration with physicians and other hospice care team members to deliver patient care and related health care services to patients and families in a variety of settings.
Responsibilities:
1. Performs medical examinations and evaluations, diagnoses, treatment, follow-up, consultation and education for both patients and family.
2. Assists with diagnosis, treatment and management of acute and chronic health conditions.
3. Orders, performs and interprets laboratory and radiology tests within scope of professional practice.
4. Writes orders pertaining to informed consents, diagnostic procedures and treatments. Prescribes medications and other treatments. Administers medications as prescribed.
5. Consults with physician or designee as needed, informs attending physician of services provided and collaborates with other physicians as needed. Facilitates clarification of patient and family goals of care.
6. Prepares and maintains accurate patient records, charts and documents to support sound medical practice and reimbursement of services provided.
7. Executes physician’s orders in accordance with legal, ethical, and organizational standards.
8. Complies with applicable laws and regulations. Initiates reimbursement for services rendered on a timely basis.
9. Ensure provision of informed consent for CPR and DNR.
10. Recommends medications as required to treat specific populations.
11. Adheres to the practice of confidentiality regarding patients, families, staff and the organization.
12. Provides educational sessions to advance hospice care among internal staff, medical community and community as a whole.
13. Participates in on-call coverage and assists in establishing schedule.
14. Participates in regular interdisciplinary team meetings to review active cases.
Other Duties and Responsibilities:
15. Adheres to internal controls established for the department.
16. Coordinates Inpatient APRN work assignments
17. Communicates potential assignment changes based on daily census
18. Reviews and coordinates Inpatient APRN PTO requests to ensure staff coverage meets current patient needs and census
19. Prepares appropriate group texts to ensure proper communication to all Inpatient APRNs.
20. Update and consult with the Chief Medical Officer on APRN staff updates when appropriate
21. Acts as point person for Inpatient APRN staff during emergency alert status
22. All other duties as assigned.
Qualifications:
23. Graduate, advanced education with Master’s degree
24. Minimum of two (2) years nursing experience.
25. Experience in hospice palliative care preferred
26. Valid State of Florida Driver’s license with driving record acceptable to Community Hospice insurance carrier.
27. Advanced Practice Nurse or PA licensed in the State of Florida
