1. To provide holistic OT assessment, goal setting, individualised treatment programmes, care planning and delivery. 2. Regular review and discharge of patients. 3. To provide standardised cognitive assessments and treatment programmes as required. 8. To provide comprehensive environmental and functional assessments and treatment programmes, including home visits and access visits. 9. To educate and provide necessary interventions around management of anxiety and fatigue. 10. To provide comprehensive manual handling care plans in a timely manner to ensure the effective and safe delivery of care plans. 11. To identify, assess and manage risk. 12. To triage new referrals, manage handovers and allocate work accordingly. 13. To contribute to interdisciplinary working by teaching and education of professional skills & techniques as defined by an agreed competency framework, both personally and for other staff and students. 14. Where appropriate to take responsibility for the application of new skills to rehabilitation programmes. 15. To provide line management and supervision to junior staff and practice placement students as appropriate. 16. To maintain excellent clinical practice through the CPD process, and taking part in relevant research, audit and development activity as directed. 17. To work closely and jointly at times with the MDT to ensure a timely, holistic and effective care plan is provided to the patient. 18. To be responsible for the day-to-day management and prioritisation of your caseload and be able to respond to urgent referrals and an unpredictable workload. This could require prolonged concentration in an unpredictable environment. 19. To be able to manage potentially stressful, upsetting or emotional situations in an empathetic manner when imparting bad news regarding rehabilitation potential. To ensure this is communicated sensitively to all parties concerned. 20. To keep yourself well at work both physically and emotionally, to access peer, clinical and management support when needed and to be aware of own limitations and escalate accordingly. 21. To report risks and harm to patients using the Trust reporting system.
