We are looking for Occupational Therapists or Physiotherapists to join our Integrated Discharge Hub. You must be able to work as part of a team, have excellent clinical reasoning and problem-solving skills.
Responsible for ensuring smooth daily operations at the Hounslow Discharge Hub based at the West Middlesex Hospital site (Chelsea and Westminster Hospital Foundation NHS Trust) and to act as a key point of contact and subject matter expert to be agreed with the postholder to support with complex discharge queries.
You will work as a core member of the acute based multi-agency Discharge Hub team to build and deliver the shared vision for a Hospital Discharge Service. Where be responsible for day to day management of the Discharge Hub; including acting as a line manager for junior staff within the team.
Main duties of the job
The post holder will be required to:
* Provide highly skilled and autonomous care to a designated caseload / wards including taking responsibility for the assessment (if needed), management and liaison (external and internal) to patients in an acute setting.
* Act as a pro-active advocate for positive risk taking in discharge planning through extensive knowledge of relevant community based pathways and ability to support effective discharge planning adopting Home First principles.
* Act as a key point of contact for acute clinicians including consultants, other members of the Discharge Hub team and community-based clinicians/services.
* Support in delivering high quality care that delivers best value and meets the requirements of the Hospital Discharge Service guidance.
* Meet the needs of the service and to support resilience and professional development, the postholder will be required to undertake some rotational work with the community-based teams.

Person Specification
Education and Qualifications
* Diploma or degree in occupational therapy or physiotherapy
* Current HCPC registration
* Evidence of post graduate training in designated speciality
* Leadership/management course
* Membership of professional body
Experience
* Minimum of 2 years work at Band 6 level in a variety of specialties
* Experience of working at a senior level in specialty designated to post
* Leadership and first line management experience
* Experience of clinical education to graduate standard in designated specialty
Skills and Knowledge
* Specialist knowledge and specialist knowledge application of current best practice in discharge planning and discharge to assess
* Knowledge of mental capacity, risk assessment and management and outcome measures relating to discharge
* Understanding and application of national guidelines and health legislation (e.g. hospital discharge guidance)
* Training and presentational skills
