Community Nursing Sister/Charge Nurse - Wokingham
* Working Hours: Monday to Friday, 8:30 am to 4:30 pm (part-time options available). Includes one weekend every four weeks, bank holidays on rotation, and one late shift (4:30 pm to 7:00 pm) per week as part of a rota.
We are seeking an experienced Community Nursing Sister/Charge Nurse to join our Wokingham Community Nursing Services team.

Main duties of the job
* Responsible for a complex and diverse caseload, actively working to prevent admission to the acute sector and supporting patients in their preferred place of care.
* Responsible for maintaining detailed, accurate and confidential patient records, adhering to Trust policies.
* Leading an effective supportive team, working together to provide clinical advice and support to other staff and patients.
* Responsible for the management of a defined caseload of patients, with continuing responsibility for the assessment of care needs, the development, implementation, and evaluation of care programmers.
* Accountable for the standards of care and for determining the activity of your team. This will include acting as a role model for staff, sharing learning and developing practice.

Job responsibilities
The must haves for you to be considered for this role:
* Adult nursing qualification along with NMC registration
* A desire to develop leadership and management skills
* A passion for community nursing, that matches our passion to provide excellent healthcare for our patients within their own home.
* Full UK driving licence, and either have access to a vehicle which can be used for work purposes or are you able to travel as required to meet the needs of the service. *Please note not all office locations or client care bases may be reached by public transport*

* Registered Nurse - Adult
* Active on the Mentor Register
* Evidence of Leadership & Management development
* Recognised coaching qualification or willing to undertake
* Non-Medical Prescriber/Nurse Prescriber Qualification
Continuous Professional Development
* Evidence of continuing professional development (CPD)
* Evidence of a current CPD portfolio
Previous Experience
* Experience in mentoring and assessing both pre and post reg. nurses in a community setting
* Evidence of clinical leadership within a community nursing environment including;
* Coaching
* Risk and Incident Reporting (Root Cause Analysis) and supporting teams in learning from findings
* Clinical Leadership course or willingness to undertake
* Able to manage own workload and support others
* Able to conduct courageous conversations and support staff with performance related issues
* Experience of leadership within a team.
