Job summary An exciting opportunity has arisen for an enthusiastic and experienced Therapy Support Worker to join the Integrated Rehabilitation Therapy Team at Camborne Redruth Community Hospital.This post will be a generic post working across Physiotherapy and Occupational Therapy so we are looking for a candidate who has experience at a Band 3 level in both of these areas, however this a supportive learning environment and we will help you to develop in both areas with competencies, our monthly in service training, and working jointly with out experienced therapists and support workers.It will also involve working mainly in the Community Rehabilitation Team but providing support to the inpatient team on the Community Assessment and Treatment Unit (CATU) when required.The community rehabilitation team work in the North Kerrier locality to provide rehabilitation to people in their own homes which also includes care homes. The CATU is a unit in transition working towards a model of reducing frailty admissions to the Acute trust by providing acute medical, nursing and therapy interventions. We are looking for a colleague who is reliable, flexible, and wishes to develop their assessment, care planning and rehabilitation skills whilst being part of a dynamic, progressive multi-disciplinary team.We have 2 part time posts available, up to 30 hours per week Monday-Friday. Main duties of the job Under the guidance of the Occupational Therapists/Physiotherapists you will be responsible for the assessment, treatment and discharge planning of patients within your care. You will need to assist in the interpretation of information relevant to the patient's condition/progress, identify problems and patient needs, setting realistic goals.You will need to have excellent organisational skills combined with experience of working in older person's services. You will need a valid UK driving licence and the ability to travel to different locations to facilitate hospital discharges.We are a supportive team, including OTs / PTs and support workers with excellent MDT working with other disciplines.
