Hope House Children’s Hospices have an opportunity for a Transition Practitioner to join our team based in Tŷ Gobaith, Conwy. You will join us on a part-time 22.5 hours, permanent basis. In return, our Transition Practitioner will receive a competitive salary of £37,088 to £44,085 per annum F.T.E.
Hope House Children’s Hospice is a centre of excellence for children’s palliative care ensuring every family facing the death of a child gets the very best care and support when and where they need it. No one should face the death of a child alone.
About the Transition Practitioner role:
We are recruiting for a Transition Practitioner, based in Tŷ Gobaith, Conwy with a minimum of 3 years working with children and young people as a registered nurse, social worker or allied health professional.
Your role will be working alongside the wider teams at Hope House and Tŷ Gobaith, and external care providers, with a focus on empowering young people and families to have their own voice to enable navigation through the adult system.
The Transition Service will support young people and their family through their teenage years up to the age of 18. Building up their confidence and ability to manage the care and support they may need in adulthood. You will work closely with young people with life-limiting conditions, and their families, to assess, plan and co-ordinate the delivery of age appropriate services to support identified needs, including Advance Care Planning.
As a highly experienced nurse, social worker or allied health professional you will be a positive role-model for your team, offer expert advice and support, pro-actively developing your team’s skills, knowledge and confidence.
Your role as Transition Practitioner will support staff, children and families at the Tŷ Gobaith hospice.
You will put the children and families at the heart of everything we do, you will have the opportunity and time to build meaningful relationships with children and their families, which makes this an incredibly fulfilling role. We are looking for a Transition Practitioner who:
* Excels in working as part of a multi professional team in order to meet the needs of the young person and family.
* Contributes positively, ensuring a streamlined transition process for the young person and family, with the young person and family at the heart of all decision making.
* Is an effective coordinator who is able to initiate, maintain communication and collaboration with external professionals.
* Is competent in carrying out comprehensive young person and family assessments, reviewing transition progress to initiate and maintain the plan of care.
* Advocates on behalf of young people and families within health care and the wider community, in doing so, supporting young people to plan for their future.
