Job summary An exciting opportunity has arisen to join the Night service Community Team at Buckinghamshire Healthcare NHS Trust Do you want the privilege of caring for patients in their own homes to deliver increasingly complex, holistic care? We are ideally looking for someone who can work autonomously in the community at Community Staff Nurse level. As a community nurse, you will work alongside a Case manager and a Community Healthcare Assistant, but at times will take the lead in triage and making care decisions; nursing patients with multiple complex health care needs in their own home.
