Do you have the skills to work in community physical health? Are you interested in working for a dynamic and innovative trust that provides person-centred health care to our local community patients? We’d love you to join our team.
We have an exciting opportunity for a Band 5 enthusiastic, motivated, and compassionate nurse to join our established multidisciplinary Care Community Team to support and deliver a range of quality nursing care to our community patients.
Your skills will be really valued by our teams and patients alike. If you are new to community health, this is a great opportunity to gain experience in this unique field of nursing.
This post is part time, 22.5 hours.
* We offer a relocation package of up to £8,000**
The relocation package is for anyone who would need to relocate in order to take up employment with Cheshire and Wirral Partnership. Terms and conditions apply.
Our care community teams are based in The Fountains Health Center covering the surrounding geographic area. It’s important that you can drive and have access to your own vehicle for work. Mileage expenses are reimbursed in addition to your hourly rate.
As a Community Staff Nurse, the successful candidate will undertake duties involving the delivery of high-quality nursing services and holistic assessments of patients, ensuring a person-centred approach.
You will work alongside various professionals to provide quality and seamless care to patients predominantly in their own homes. You will be proactive in coordinating their care needs as appropriate, avoiding unnecessary admissions to hospital or long-term care settings.
