* 2 References - Where possible we require 3 years' employment history.
* Qualification - Social Work Qualification and registration with Social Work England.
* Essential Car Documentation - This post is a designated Essential Car User. Therefore, you must always hold a full valid driving licence and provide your own car for use at work. You will receive the Essential Car User Allowance.
* DBS (Disclosure and Barring Service) Clearance - This post is covered by the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (amended in 2013). Appointable candidates will be required to apply for an Enhanced Disclosure and Barring Service (DBS) check.
* Evidence of conduct in previous employment concerned with the provision of services relating to Health or social care or Children or vulnerable adults.
* Proof of identity including a recent photograph.

We are looking for motivated, confident and experienced Social Workers in a fast-paced MDT team working within the Hospital@Home service as part of the HDTA Community Service. This is a busy and focused team which involves critical thinking and the ability to manage risk and need in partnership with Health colleagues within the H@H service. The role involves assessment and support planning for citizens receiving clinical care and assessment in their homes under the H@H service. The role will suit Social Workers able to work at pace in collaboration with others to support citizens discharged from Hospital under the H@H service.
About The Candidate:
The ideal candidate will be a qualified, experienced and confident social worker able to work closely with health colleagues and able to complete comprehensive Strengths based Assessments for people who may be in need of long-term care. Candidates would benefit from strong communication skills, the ability to challenge in the best interest of citizens and able to manage a fast-paced caseload. Successful candidates will have access to regular formal and informal supervision and will be confident in working within a wide MDT of health and social care staff.
