Community Nurse - East Area Evening & Overnight Nursing Service
We currently have a vacancy for Band 5 Community Nurse within the East Area Evening Nursing Service working 15 hours per week. The Evening Nursing Service works across Wrexham and Flintshire counties and is based in Wrexham. The shift commences at 6.00 pm - 11.00 pm, although there will be a need to be flexible to support the Overnight Service as required.
Informal visits are welcome. Community experience is desirable.
Main duties of the job
The team responds to both planned and unplanned nursing needs, i.e., palliative care, catheter care, and supportive nursing visits to monitor patient stability and provide support to both formal and informal carers. The aim of the service is to provide high-quality nursing care during out-of-hours periods and reduce the need for unnecessary hospital admissions.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

* NMC Registered Nurse
* Evidence of post-registration study and training, e.g., POVA, Child protection, intermediate life support
Experience
* Experience of providing holistic nursing care
* Clear understanding of clinical governance framework
* Implement evidence-based practice
* Clear understanding of contract of care
* Ability to document details clearly and accurately
* Clear understanding of consent process
* Basic IT skills
* Knowledge of incident reporting policy
* Awareness of policies
