Creative Support is one of the largest national non-profit providers of personalised care and support to people with learning disabilities, mental health needs, autism spectrum conditions and older people.
We are looking to recruit a resourceful and enthusiastic support co-ordinator to lead an innovative supported living service in Sheffield. You must be warm, caring, and totally committed to providing effective, active support to 5 services and providing support to a number of shared properties across Sheffield. You will embrace the opportunity to combine leadership and management with holistic, personalised support, which includes delivering dignified personal care. You will need to have a proactive attitude in identifying opportunities for interesting and varying age appropriate activities, and have a positive approach to risk management.

Articulating and reinforcing the vision of a positive, person centred supported living model, and ensuring clients receive individualised, person-centred support, devised in response to the identified needs, preferences and aspirations of individuals.
Identifying and accessing opportunities for social inclusion with clients to enable them to enjoy a fulfilling and valued life, participate in their community, attain new daily living skills and build on existing ones.
Supervising a team of support workers to achieve the highest standards of customer care and positive outcomes. Deploying staff resources effectively to meet the needs of the service.
Assisting the Service Manager in ensuring that the service meets and surpasses CQC standards and the contract specification.
You will be expected to work flexibly doing shifts on a rota, according to the needs of the service. This will include participating in local on-call responsibilities, working sleep-ins, evenings, weekends, and bank holidays.
