Due to the expansion of our team, we are looking for an experienced Practice Nurse to work with us at our friendly surgery. We are looking for a candidate full of enthusiasm to join our nursing team of 3 Practice Nurses, a Health Care Support Worker, and a Phlebotomist. The role will be varied and cover all aspects of Practice Nursing including chronic disease management; however, applicants looking for a role doing specific areas of practice nursing are welcome.
We offer a competitive rate of pay linked to qualifications and experience. Hours are negotiable to suit the applicant and needs of the practice (full or part time).
Main duties of the job
* To assist with chronic disease management
* To provide a holistic nursing service to all patients attending the surgery
* To deliver a range of services to agreed practice guidelines/protocols
* To assess patients' physical, psychological, and social needs
* To deliver health promotion activities to the practice population e.g., travel health, sexual health, and immunisation
* To manage minor trauma, acute and chronic wounds.
* To help maintain the disease registers in line with the local and national guidelines
* To provide lifestyle advice and screening to aid in the prevention of possible future illnesses

Job responsibilities
* Chronic disease reviews
* Assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs
* Cytology
* Wound care
* Removal of Sutures
* ECGs
* Asthma/Peak Flow Readings/Spirometry
* Venepuncture
* Ear syringing
* Routine immunisations/Childhood immunisations
* Chaperoning and assisting patients where appropriate who are being examined by another clinician
* Assisting GPs with minor surgery and coil fittings
* Requesting pathology tests, for example urine culture, swabs
* Providing contraceptive advice/pill checks.
* Following agreed clinical protocols with referral to senior nursing staff or GPs as appropriate
* Ordering of vaccinations to maintain stock levels
* On occasion, there may be a requirement to undertake home visits
* Any other delegated duties appropriate to the post
Person Specification
Experience
* Experience of chronic disease management
Qualifications
* NMC Registration
* IT literate
* Diabetes or Asthma/COPD diploma
