Care Coordinator
Are you an experienced Care Coordinator or Field Care Supervisor within domiciliary care who thrives when working for a loyal and supportive company? Our client has established successful businesses within social care recruitment, and they have just opened their first domiciliary care service. They're looking for somebody to support the Registered Manager with the day to day running of the service.
As the Care Coordinator, you'll essentially be responsible for supporting a team of care staff who deliver care in the community, ensuring that care and support is delivered in a professional manner and that all documentation is accurate and kept up to date. It'll be a varied role where you'll spend time both in the office and out and about in the community.
On offer is an attractive salary of £25,000 - £27,000 per annum.
The Care Coordinator Role
1. Organise the staff team, and support them with training and development needs.
2. Drive improvements in care delivery by observing, mentoring and upskilling carers.
3. Carry out competency assessments and care audits.
4. Maintain electronic records such as Care plans, Risk Assessments and Client Reviews.
5. Complete medication assessments.
6. Work with the rota organiser to ensure that the care calls are scheduled in an effective manner.
7. Have a good understanding of clients' needs so that you can respond to changes responsively.
8. Liaise with stakeholders such as brokerage teams, healthcare professionals and clients' families.
About You
1. We're looking for somebody who has experience within domiciliary care in a leadership position such as Field Care Supervisor, Care Coordinator, Care Manager, Care Team Leader or similar.
2. You'll have great organisation skills and be able to work flexibly to meet the changing needs of the service.
3. You'll have QCF level 3 or above in health and social care.
4. You'll demonstrate a caring and respectful attitude and be committed to personal growth and development.
