Collaborate with physicians and nursing staff to develop and implement patient-specific medication plans. Monitor patient responses to medications, adjusting treatment plans as necessary based on clinical assessments. Educate patients and their families about medication use, potential side effects, and adherence strategies. Maintain accurate records of medication administration and patient interactions in compliance with regulatory standards. A degree in Pharmacy from an accredited institution. Registration with the General Pharmaceutical Council (GPhC) or equivalent regulatory body. Excellent communication skills, with the ability to work effectively within a multidisciplinary team. A commitment to providing high-quality patient care and continuous professional development. Demonstrates use of appropriate communication to gain the co-operation of relevant stakeholders (including patients, senior and peer colleagues and other professionals, other, e.g. hospitals).
