Job Description To be responsible for the production of information on performance and quality standards within the practice. To manage data quality issues, providing advice and guidance to all staff in the use of clinical systems. To perform complex data analysis and support the interpretation of clinical data, using statistical techniques. To analyse and interpret complex data and assist with planning and monitoring work undertaken in relation to Directed National and Local Enhanced Services (LES), the Quality Outcomes Framework (QOF) and the Quality Improvement Scheme (QIS) taking appropriate action to achieve targets and accurate claims are returned. To deliver complex information to a varied audience appropriately, ensuring widespread understanding. To deliver accurate information on time to internal and external stakeholders using appropriate analytical and presentation techniques. To be the main lead on Quality Outcomes Framework (QOF). To work with the Practice team and to review key performance indicators in the Quality Outcomes Framework (QOF) and Quality Improvement Scheme (QIS). Work strategically to review and monitor patient call and recall systems. To provide monthly updates to staff in relation to practice performance of key targets and indicators. To contribute to the delivery of a diverse range of information enquiries generated by internal and external stakeholders. Essential criteria To be aware of and promote national, local and practice quality standards for chronic disease management. Ensuring that quality of care is maintained at a high standard using NICE Guidance and the existing GP contract as a guide. To oversee the processes required to achieve standards for; - Quality Outcomes Framework (QOF) - Quality Improvement Scheme (QIS) - Enhanced Services - Any other initiatives asked of us from external stakeholders To work with colleagues and stakeholders to develop performance monitoring, quality improvement, benchmarking, forecasting and modelling tools that accurately reflect and record practice targets of clinical data. To provide support in targeting patients for assessment and treatment including patient care plans for housebound and frailty etc. Ensure staff are aware of the importance of maintaining disease registers and assist in the validation process. To provide advice and support for clinical staff and produce regular reports to assist progress of these groups to achieve targets using appropriate presentation techniques that are easily understood by staff, and to highlight issues, trends and areas of interest. To assist in the production of information for clinical audits as requested by the clinicians and make recommendations to complete audit cycles. To assist researchers in achieving their objectives in conduct of research within the practice. To provide support and training for current staff and new staff ensuring data quality guidelines are understood and adhered to. To oversee all methods of data entry ensuring templates and protocols are reviewed and updated as required. Undertake any other additional duties correlating with the role as requested by the Partners and Senior Management. Ensure Smartcards are not left unattended and are stored securely overnight.
