What you will do: Conduct Care Act Assessments, Mental Capacity assessments, and facilitate Best Interest outcomes. Complete Community Deprivation of Liberty assessments and engage in Continuing Healthcare (CHC) checklists and Decision Support Tool (DST) processes. Operate from a Strengths-Based perspective, focusing on Least Restrictive Practice. Support the team by triaging referrals and prioritising day-to-day case management activities. Collaborate confidently with professional partners within a multidisciplinary team (MDT) approach.
