Job summary Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population, delivering care within their scope of practice, including to housebound patients. The Practice Nurse will be responsible for several clinical areas such as infection prevention and control, health promotion, chronic disease management, NHS Health Checks, as well as actively supporting the practice management team in the reviewing and delivery of clinical policy and procedure. An interest in diabetes is desirable. Prescribing qualification is also desirable but not essential (pay scale will reflect prescribing qualification). St Gabriels Medical Centre will ensure that the post holder has access to appropriate clinical supervision and an appropriate named individual in the organisation to provide general advice and support on a day-to-day basis. There are bi-weekly support meetings with a lead GP.
