We are based in the community and provide patient-centered care to patients in their own homes. Your job would be to assess patients at home for Occupational Therapy interventions and consider ongoing needs of each patient and identify opportunities for longer-term support where required.
We work a 7-day shift pattern from 08:30 to 16:30.
We cover a large geographical area, so access to a vehicle to travel to visit patients is essential.
Main duties of the job
We are an evolving service with structured leadership and support. We are looking for a dynamic occupational therapist to join us to support patient discharges home from the hospital and to create a fantastic community rehabilitation service. Patient care is the heart of what we do within our 3 community teams in South Wiltshire, and our vision is to provide outstanding care for every patient. You'll work within a supportive multidisciplinary team with a management structure that welcomes innovations and quality improvement whilst prioritizing staff well-being. Leadership is given in the form of therapy and nursing clinical leads, the team manager, advanced clinical practitioners, and consultants as part of our virtual ward approach prioritizing keeping patients well at home. You will work alongside community-based physiotherapists and together support the rehabilitation support workers who assist the patients at home.
Here in Salisbury, we are based at the 5 Rivers Leisure and Well-being Centre, just off the ring road into Salisbury. There is free car parking, and a permit to support your community visits. Travel costs are reimbursed for travel within your working day. The swim, fitness, and gym leisure facilities are onsite and can be accessed with a corporate discount, and there is also a café onsite too. We support flexible working and will consider requests, considering the needs of the service.

Person Specification
Qualifications
* Dip COT/BSc/BSc(Hons)/MSc OT
* Registered as an OT with the Health and Care Professionals Council
* Non-medical prescriber
* Leadership and Management training
* Further qualifications relevant to community practice
Experience
* Up-to-date community experience
* Significant experience of working with adults
* Working with a multi-disciplinary team
* Teaching experience
* Knowledge of recruitment, selection & appraisal system
* Setting and monitoring standards of care
* Experience within a wide range of patient/client care groups
* Profiling community needs and targeting identified health needs of the population
* Evidence of innovative and flexible approach to care and the organization of care
Additional Criteria
* Access to a vehicle that can be used for business purposes and hold a full and valid driving license.
