We are looking for you to work as as part of the Amersham Rapid Response & Intermediate Care team supporting patients in their own homes, providing personal, social and rehabilitative care. The overall aims of the Rapid Response & Intermediate Care Team are to avoid inappropriate admission of patients into Hospital and facilitate timely discharge into the community.
The team is committed to ongoing education on the job and development opportunities are available within the organisation. Successful applicants If who don't already have a Level 3 Qualification in Health and Social Care will be expected to work towards that in post.

Your main duties include:
• Providing general and specific care to adults within competencies
• Being an integral member of the Rapid Response & Intermediate Care (RRIC) team, providing defined delivery of nursing, social and rehabilitative care to patients in liaison with senior staff.
• Taking responsibility for designated elements of the patient treatment protocols and delivery of such treatments in patients’ own homes.
• Being responsible for the completion of delegated non-clinical tasks within the team.
Your main responsibility is to :
• Improve the care of patients in the community, so patients can remain safely in their own home for as long as possible
• Have an innovative approach, excellent communication skills and willingness to work with other members of the community teams to ensure high quality service delivery
• Willingness to further develop your clinical expertise in a range of areas
A full UK driving licence and access to a car with Business insurance for work purposes
