Job overview
To work as an integral part of the Flow Admin Support Team, supporting the development and provision of digital services to ensure the needs of the patients are met.
Work closely with case managers, patient flow team, and wider MDT in the assessment/ discharge of patients both within and external to the organisation in line with Government Guidelines.
Act as the administrator coordinator for the patient flow team in the absence of senior colleagues.
Provide educational support and training to junior members of the Patient Flow Team.
Undertake assessments to assist in timely discharges.
Be able to identify suitable pathways and appropriate services to meet patient need in hospital and in community setting.
Main duties of the job
1. Work in collaboration with the Team Leader, and other members of the Flow Team.
2. Triage referrals with the support of the case managers and social care colleagues.
3. Independently undertake assessments and interventions relevant to specific pathways.
4. Supervise junior staff and provide support to other team members and students.
5. Work flexibly according to patient and service needs.
6. Manage, coordinate and prioritise the workload within Patient Flow Admin support team.
7. Liaise with, and make appropriate referrals to other agencies.
8. Clearly document each intervention in the patient's records as outlined in organisational policy and procedures.
9. Support and prevent unnecessary admissions to hospital where possible.
10. Work in collaboration with other agencies to develop pathways of care for individuals to facilitate early discharge from hospital or prevent unnecessary admissions to hospital.
11. Contribute to the planning, implementation, monitoring and review of referrals for individuals with the members of the Flow Team and ensure a smooth transition of care to community providers.
12. Work in partnership with patients, carers, the MDT and wider statutory, private and voluntary sector services.
13. Good communication skills to establish good working relationships with colleagues, service users and carers.

SPECIAL KNOWLEDGE/ EXPERIENCE/ COMPETENCIES
* Knowledge of / use of SystmOne, Experience in use of IT applications.
* Demonstrate an understanding of the role of intermediate care/rehab/ palliative care and telemedicine services in the community.
* Experience of working in services caring for Acutely ill patients/complex care needs.
* Experience of working as part of a clinical team.
* Understanding of interagency working.
* Experience of supervising and supporting others.
* Community care experience.
* Experience of working with long term conditions and frail elderly.
Qualifications
* Diploma in a relevant health/social care topic/working towards.
* NVQ level 3 Health/Social Care or equivalent experience.
* Good general level of education including GCSEs in English and Maths.
* Courses relevant to service/clinical area.
* ECDL or formal qualifications in word processing and spreadsheets.
* Teaching/Assessing qualification.
PRACTICAL AND INTELLECTUAL SKILLS
* Demonstrate competent communication and interpersonal skills.
* Demonstrate level of self-awareness, i.e. strengths and weaknesses, personal qualities and skills.
* Ability to utilise information to inform practice and service need.
