The post holder is a Care Coordinator, who acts within their professional boundaries, working alongside the PCN multi-disciplinary team, general practice teams, and within the Primary Care Networks. They work for the benefit of patients, providing and maintaining a high standard of care for patients' health needs. The post holder will be an integral part of the General Practice team, building relationships with other community providers, community services, and working closely in partnership with other staff within the PCN and practices, including other Social Prescribing Link Workers.
Key Responsibilities:
* To work in a patient-facing role, with the GPs and other primary care professionals to identify and manage a caseload of patients.
* To assist the practice with summarising and coding of patients' records.
* Identify, where required and as appropriate, patients who need to be referred back to other health professionals within the PCN.
* Provide coordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers, health wellbeing coaches, and other primary care professionals.
* To support patients to utilise decision aids and help create single personalised care and support plans, in line with best practice.
Main Duties of the Job
* To help people manage their needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care.
* Assist people to access self-management education courses, peer support, or interventions that support them in their health and wellbeing.
* To support the coordination and delivery of Multi-Disciplinary Teams within the Primary Care Network working with the Collaborative Care Team.
* Raise awareness of how to identify patients who may benefit from shared decision making and support PCN staff and patients to be more prepared for shared decision-making conversations.
* Provide the interface between primary care and other services, helping patients with the transition between services.
* Ensure awareness of statutory and local clinical protection procedures, including systems of referral.
* Attend and participate in any practice meetings when required.
* Any other delegated duties appropriate to the post.
* This post is a PCN role, so you may be required to work at other PCN practices.

Experience
* Experience of working in a busy, patient-facing environment; experience of working with I.T systems i.e. Microsoft Office and/or Clinical Systems.
* Knowledge and an awareness of common acute and long-term conditions that are likely to be seen in general practice.
Skills and Abilities
* Ability to communicate complex and sensitive information effectively with people at all levels by telephone, email, and face to face.
* Ability to follow legal, ethical, professional, and organisational policies/procedures and codes of conduct.

£12 an hour, negotiable and depending on qualifications and experience.
