Our Directorate of Planned and Proactive Care: Community services is seeking a Band 5 RGN to support the Community Cardiorespiratory teams in North Nottinghamshire; Bassetlaw. This is a really exciting opportunity to be part of a specialised community team.
To be pro-active in delivering anticipatory and responsive care, promoting independence, health lifestyle, and reduction in health inequalities to improve the overall quality of life in the local population for cardio-respiratory patients.
To offer a comprehensive nursing / therapy service that is a multi-disciplinary and multi-agency approach to maximise patient function, well-being, and self-care planning.
To work collaboratively with the respiratory physiotherapists in the planning and delivery of pulmonary rehabilitation programmes.
To support ongoing service development of Adult Community Health Services within Bassetlaw to achieve a model of wrap around care that is responsive to the needs of the patients / service users.
As the Band 5 Cardio-respiratory nurse you will:
1. Support with specialist clinical assessment of people with Heart failure and COPD/ Respiratory conditions.
2. Develop skills and competencies in health assessment, medicines management, and case management to support patients with Heart Failure COPD / respiratory conditions.
3. Work collaboratively and integrate with primary, secondary, social care, the voluntary sector and with patients to establish a whole systems approach to managing health and social care for the designated patient group.
4. Provide support to both students and other professionals.
5. Work within Pulmonary Rehabilitation and undertake pre/post assessment and individualised Exercise programs for patients.
6. Support and educate patients and carers about their Heart failure and COPD/Respiratory condition so they are equipped as far as possible with the knowledge and skills to manage their condition to their maximum potential.
