Location: Midlothian or East Lothian
Salary: A GBP 29,616 per annum full:time (pro rata for part:time)

The Role
Our client is looking for full time and part time Practitioners to join their Wellbeing teams located in Midlothian or East Lothian GP Practices.
* Are you a health and social care practitioner with experience of supporting people living with long term physical and mental health conditions to self:manage?
* Do you cope well supporting vulnerable people who experience significant psychosocial barriers and health inequalities?
* Do you believe people have strengths, are resilient and are experts in their own lives?

In East Lothian area they are recruiting for the equivalent of one full:time post of 39 hours per week and one post of 31.2 hours per week (4 days). Whilst practitioners will be predominantly based in Musselburgh practices (Inveresk and Riverside) and/or Prestonpans and Tranent, travel between various practices across the area is essential.
In Midlothian they are recruiting for the equivalent of one full:time post of 39 hours per week : travel to more than one GP practice is essential. They will consider interest in full or part:time hours. Preference for hours, work pattern and location should be clearly stated in your cover letter.
Key Responsibilities
* Deliver holistic one:to:one and group based self:management support to people living with long term health condition(s) across Midlothian or East Lothian
* Work with a team of practitioners to deliver their Midlothian or East Lothian GP contract
* Practice persistent engagement to help people who struggle to receive support
* Support volunteer/paid peer supporters who have lived experience of long:term conditions and are supporting the Health and Wellbeing team.
* Use a monitoring, evaluation and reporting framework to learn and develop practice
* Develop and maintain links with referrers and services and supports they signpost people
* Contribute to the Improving the Cancer Journey Programme (ICJ) which is funded by Macmillan Cancer Support as part of the Lothian:wide Improving the Cancer Journey Programme.
