We are looking for motivated and driven individuals with good values and a genuine interest in the care and support of older people with experience of care within a highly complex yet rewarding environment. The successful candidate will undertake the keyworker role, this will involve creating, monitoring, and reviewing care plans and keeping accurate records of support planning as well as, ensuring the service user is being offered the opportunities, experience and the care and support within their care plan. The role will also involve promoting the service users to access opportunities in their local communities and enabling them to continue to live a meaningful life. We provide ongoing care and support to vulnerable older people: the care homes also support a respite, STAR unit and day service.
