Job summary We have an exciting opportunity for a fixed term, Band 6 Community Learning Disability/Mental Health Nurse role, in the Trust-wide Young People's Specialist Support Team. This multiagency team consists of clinical psychology, learning disability nursing, speech therapy, occupational therapy and support workers. The Team sits alongside CCLDT and CAMHS, and works as a step up service beyond local community provision. The Team work with a small number of young people with severe learning disabilities and/or autistic young people with complex needs including behaviours of concern, whom require intensive support. Working alongside local respite and short breaks services, the Team work to understand the young person, their interests, strengths and needs, to create, instigate and support person-centred positive behaviour support plans to improve quality of life and reduce restrictions and restraint. This is a chance to help to deliver a service based on the principles of the Ealing Model, including positive behavioural support and trauma informed interventions. You will support the Team with the continued mobilisation of this model across our five localities, linking to the dynamic risk register and working alongside families.
