To act as an autonomous practitioner working independently and in conjunction with other health care professionals to assess, diagnose and treat the conditions of patients attending within primary care initiating direct referrals as appropriate. The ANP will be able to independently prescribe appropriate medication, evaluate or refer to other specialists if necessary. The ANP will work collaboratively with the general practice team, to meet the needs of patients and support the delivery of policy and procedures, and provide nursing support. Provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence. To develop new and innovative concepts, models, methods and practices to deliver new and improved care services to meet the needs of service users. To provide education and training to other staff and students. To undertake audit and education as part of the role., You will be an experienced nurse / paramedic who can provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. You will demonstrate safe clinical decision making. You will work collaboratively with the multi disciplinary general practice team to meet the needs of patients and providing nurse leadership as required.

* Prioritise / triage patients presenting with a range of acute and chronic conditions, making any necessary referrals for investigations in the appropriate manner
* To visit patients in their own home when required, to assess, diagnose and treat accordingly
* Implement / evaluate individual specialised treatment plans
* Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence
* Undertake the collection of pathological specimens/request pathology services as necessary
* Process / interpret pathology and other test results as required
* Maintain accurate clinical records in conjunction with extant legislation
* Ensure read codes are used effectively
* Prioritise health issues and intervene appropriately
* Support the team in dealing with clinical emergencies
* Be an extended and supplementary prescriber, adhering to guidance
* Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing as required

We have a clinical team comprising of 2 GP Partners, 2 salaried GP's, 2 ANP's, 3 Practice nurses, HCA and non-clinical team including secretary and reception/administrators. We are a small and friendly practice with strong work ethics.
