Job Summary
The post holder is responsible for the delivery of general practice nursing care to the whole practice population. Key elements of the role are the delivery of evidence-based practice for patients presenting with an immediate nursing need or a long-term condition and the provision of preventative health care to the practice population.
They will demonstrate safe clinical decision-making and provide expert care in the management of patients. The post holder is expected to work collaboratively with all members of the practice team to meet the needs of patients and support practice objectives, policies, and procedures. The post holder will work within their scope of practice and in accordance with local policies and procedures and the NMC Code of Professional Conduct.
Main Duties of the Job
* Assisting in and performing routine tasks related to patient care as indicated by patient need - including but not limited to wound care, Doppler, removal of sutures/clips, urinalysis, chronic disease management, venepuncture, routine immunisations and childhood immunisations.
* Following agreed clinical protocols.
* Upholding the standards required for Infection Prevention and Control.
* Monitoring vaccine fridge temperatures.
* Maintaining clinical rooms, stocking and rotating items as required.
* Ordering of vaccines to maintain stock levels, if required.
* Maintaining accurate records.
* Attending and participating in any practice/clinical meetings.
* Undertaking home visits, if required.
* Supporting income generation for the Practice as relevant to the role (e.g. QoF).

Job Responsibilities
Clinical
* Assist in and perform routine tasks related to patient care as indicated by patient need including cytology, wound care, removal of sutures/clips, new patient medicals, travel clinic, asthma and COPD management, venepuncture, hypertension management, diabetes management, ear syringing, routine immunizations, chaperoning, requesting pathology tests.
* Following agreed clinical protocols and maintaining and cleaning equipment used by the nurses and GPs.
* Vaccine fridge temperature monitoring and maintaining GP and Nurses rooms, stocking and rotating items as required.
* Maintaining accurate records and advising other team members of potential problems or errors.
* Attending and participating in any practice/clinical meetings monthly.
* Supporting and managing the QoF programmes and income generation for the practice relevant to role.
* Any other delegated duties appropriate to the post.

* Utilise and demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment.
* Communicate effectively with patients and carers, recognising the need for alternative methods of communication.
* Maintain effective communication within the practice environment and with external stakeholders.
Delivering a Quality Service
* Recognise and work within own competence and professional code of conduct as regulated by the NMC.
* Produce accurate, contemporaneous and complete records of patient consultation.
* Deliver care according to NSF, NICE guidelines and evidence-based care.
Leadership
* Support the lead nurse/ANP in the delivery of practice nursing services ensuring that the needs of the patient are a priority.
* Take responsibility for own learning and performance including participating in clinical supervision.

* Work as an effective and responsible team member, supporting others and exploring new ways of working.
* Delegate clearly and appropriately, adopting the principles of safe practice.

* Manage and assess risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients.
* Monitor work areas and practices to ensure they are safe and conform to health, safety and security legislation.

* Use technology and appropriate software as an aid to management in planning and monitoring of care.
* Review and process data using accurate Read codes.

* Undertake mentorship for more junior staff, assessing competence against set standards.

Person Specification
Qualifications
* Registered Nurse.
Experience
* Registered Nurse with experience of working as a qualified Nurse.
