The All Age Continuing Health Care (AACHC) team are seeking motivated, passionate and enthusiastic Registered nurses with a can-do attitude to join our dynamic team. The opportunity to work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the CHC Appeals Processes.
As a Senior CHC Appeals Nurse you will be responsible for the assessment of Continuing Healthcare patients following an Appeal. You will use your knowledge and clinical skills to ensure that outcomes are determined in a consistent manner to ensure effective application and interpretation of the National Framework for NHS CHC.
You will also review applications for review of Periods of Previously Unassessed Care (PUPoC) and ensure robust, evidence-based assessments are completed.
You will be:
* An experienced clinician with knowledge of the CHC National Framework, and a clear understanding of the CHC Appeals Process and PUPoC guidance.
* Experienced working within a team.
The team has strong links with community services, and collaborative working within the multi-disciplinary team and with our community colleagues in health and social care is essential.

Main duties of the job
We are a clinically led and supportive team working with some of the most vulnerable individuals in our community. There is an opportunity to work with colleagues in the CHC assessment teams, and the wider health and social care system to deliver the best outcomes for individuals who have been assessed for CHC.
The post holder will work jointly with health, social care, and independent sector colleagues to autonomously gather evidence and information to populate the assessment.
To follow the process laid out in the National Framework for NHS Continuing Healthcare; to include organisation, co-ordination, and participation in MDT eligibility recommendations to be verified by ICB for both Appeals and PUPoC requests.
Ensure assessments conducted are robust, transparent, and multi-agency (where required) for NHS Continuing Healthcare/NHS-funded Nursing Care.
The post holder will be responsible for a complex clinical caseload comprising of Appeal and PUPoC cases.
* The post holder will review CHC appeals and CHC PUPoC requests to ensure that the NHS Continuing Healthcare processes have been adhered to.
* The post holder will work as part of the CHC Appeals Team and will be responsible for providing clinical expertise on reviewing both Appeals and previously unassessed and assessed periods of care (PUPoC).

Detailed job description and main responsibilities:
* The post holder will be expected to produce comprehensive reports, and Needs Portrayal documents identifying a patient's journey throughout the period of review for both Appeals and PUPoC.
* The post holder will be expected to have a good understanding of Continuing Health Care and high-level communication skills to Chair Local resolution meetings with claimants and their representatives.
* The post holder will play a key role in supporting the Continuing Healthcare service across the BSOL region, working within the remit of the National Framework for Continuing Healthcare and PUPoC Guidance.
* The post holder will deliver an effective and competent level of service and consistently deliver a client-focused service.
* To work autonomously in managing a caseload of patients as directed and to accept case coordinator responsibilities.
Person Specification
* Registered Nurse with a current NMC registration or equivalent.
* Previous experience of undertaking CHC assessments.
* Conversant with national and local guidance on NHS Continuing Health Care/NHS Funded Care for Adults Continuing Healthcare and with detailed understanding of the correct use of the national tools (checklist, DST, Fast Track).
* Educated to degree level or equivalent level of experience working in a specialist area.
* Extensive nursing skills in Adults with complex healthcare needs.
* Strong verbal and written communication skills.
* Conversant in CHC Appeals Best Practise Guidance and current PUPoC Guidance.
* Knowledge of Commissioning arrangements in CHC.
* Demonstrate previous experience in progressing CHC Appeals and PUPoC.
* Evidence of having undertaken leadership role in working across boundaries.
