Job Overview
We are seeking a dedicated and compassionate Care Coordinator to join our team. The ideal candidate will play a crucial role in ensuring that our clients receive the highest standard of care. This position involves coordinating various aspects of client care, liaising with healthcare professionals, and providing support to clients and their families. The Care Coordinator will be responsible for maintaining accurate records and facilitating communication between all parties involved in the care process.
Duties
1. Coordinate and manage client care plans, ensuring that all services are delivered effectively.
2. Liaise with healthcare providers, social workers, and other professionals to facilitate comprehensive care.
3. Maintain accurate and up-to-date records of client interactions, care plans, and progress notes.
4. Provide support and guidance to clients and their families regarding available services and resources.
5. Conduct regular follow-ups with clients to assess their needs and satisfaction with services provided.
6. Assist in the development of policies and procedures related to client care.
7. Assist in the production of staff rotas and staff training and supervision.
8. Assist the Registered Manager wherever they are required to do so.
9. Participate in team meetings to discuss client progress and any necessary adjustments to care plans.
Requirements
1. Previous experience in a healthcare or office setting is preferred.
2. Excellent organisational skills with a keen attention to detail.
3. Strong communication skills, both verbal and written, with the ability to empathise with clients.
4. Ability to work independently as well as part of a team.
5. Proficient in using office software applications for documentation and communication purposes.
6. A proactive approach to problem-solving and the ability to handle sensitive information with confidentiality.
7. A commitment to ongoing professional development in the field of healthcare coordination.
