Ranworth PCN has a fantastic opportunity for a healthcare assistant to join our proactive ambulatory care team at the Kennedy Way Medical Centre in Clacton on a full-time basis for 37.5 hours per week, Monday to Friday. This role requires the successful applicant to see all patients at their home or care home, therefore a driving licence and own transport are essential. The proactive ambulatory care model is part of a broader shift within the NHS toward more community-based, patient-centred care that aims to keep individuals out of hospitals and support them in managing their health conditions more effectively at home. The Ranworth PCN Proactive Ambulatory Care Team is a healthcare initiative aimed at improving patient care by providing proactive, out-of-hospital treatment and support, particularly for individuals with complex or long-term health conditions registered with us as housebound or resident in a care or residential home. We have around 900 housebound patients and 450 care home beds. This approach is designed to prevent hospital admissions and reduce the need for emergency care by managing conditions in the community and enhancing patient outcomes. You will support the proactive ambulatory care team in the delivery of nursing services to our housebound and care home patients, delivering care within their scope of practice to the entitled patient population. You will assist clinical staff in the provision of treatment, preventative care, health promotion and patient education. The main responsibilities will include: Undertake new patient health checks. Support the practice nurse with health promotion programmes. Carry out baseline observations such as pulse oximetry, blood pressure, temperature, pulse rate, height, weight and BMI, recording findings accurately. Carry out annual HCA YOC PT 1 diabetic checks in line with guidance. Facilitate routine and 24-hour BP monitoring, advising patients accordingly. Undertake wound care, dressings and other clinical tasks as required. Carry out ECGs as requested. Provide support during minor operations as required. Ensure all clinical rooms are adequately stocked and prepared for each session. Maintain surgical equipment. Complete home visits as required for patients requiring any of the above. About the Candidate If you are a qualified and experienced healthcare assistant looking for a fresh and exciting challenge, we would love to hear from you. You will have the following skills and qualifications: Level 3 or 4 Diploma in Health and Social Care, or equivalent. Ideally a qualification in phlebotomy/venepuncture. Strong clinical skills. Experience of working with the general public within primary care or another healthcare setting. Confidence and competence in all HCA duties (as listed above) required for the post Excellent communication and interpersonal skills. Consistently confidential manner, as well as a compassionate and respectful approach, placing a high value on the dignity of patients, relatives, carers and colleagues. Ability to work independently and determine own workload priorities, as well as working as part of a multi-disciplinary team. Strong organisational and time management skills.
