Main duties of the job To ensure facilitation of safe discharge needs of the complex patient work load inclusive of the Neo-Natal Unit and Paediatric A&E/PAU as required. Develop, implement and evaluate service discharge policies and protocols to ensure appropriate services receive information of children who have had an in- patient episode or attended Emergency Departments irrespective of the reason for admission or attendance. Participate in assessment, planning and provision of patient's needs before discharge. Ensure teaching and development of parents/carers and nursing staff. To ensure EPUT Community Services adheres to the Pan Essex Guidance for Managing Emergency / Unplanned Care Liaison Information. To be the named single point of access to children's community services for Southend University Hospital Foundation Trust (SUHFT) for children. To represent EPUT at agreed multi-agency / disciplinary forums, discharge planning meetings/ward rounds as required to meet the remit of this position.
