This exciting role will provide clinical pharmacy services to Community Hospitals mainly based in Bodmin and/or Camborne Redruth or west of cornwall area.
Working pattern can be discussed and various options will be considered for the right candidate which will include a rolling weekend rota. Ideally, we would like to recruit to a band 5 position, but that we would consider a band 4 to 5 progression pathway for the right candidate.
This is a varied role with the successful applicant needing to work independently and within the wider pharmacy team. As a Medicines Optimisation Pharmacy Technician you will be required to work across physical and mental health inpatient teams to support with medicines reconciliation, prescription reviews, medication recycling, discharge facilitation, ward-based dispensing, patient counselling and auditing.
Main duties of the job
To provide a comprehensive medicines management service to patients on these wards which includes medicines reconciliation, prescription review and endorsement, recycling of medication, supporting self-medication and, where necessary, patient and carer counselling.
To participate in multi-disciplinary activities such as attending ward handover meetings and facilitation and preparation of discharge medication including the handover of information regarding a patient's medication to other healthcare professionals if required.
To work with the pharmacy team in delivering clinical pharmacy and safe systems of working with medicines across other community hospital and mental health sites.
To supervise and support with education and training activities such as student technicians, student pharmacists and pre-registration pharmacists
To specialise if the opportunity arises as per service needs and personal interest (for example, Parkinson's, Medicines Safety)
This post will support hospitals in Bodmin, and will be required to travel to other peripheral hospitals including but not limited to Liskeard, Stratton, Launceston and Camborne Redruth to provide cover.
