Community Mental Health Recovery Service Reigate is seeking enthusiastic community mental health nurses (NMC) to join the team.
To work within a community team based within the area, working in partnership with statutory and voluntary sectors to facilitate a whole system approach to care delivery. The Post Holder will act as Care Coordinator/Lead Professional, where appropriate, to patients who require the skills of a Mental Health professional, and provide a holistic, clinical autonomous nursing service, to enable individuals to engage in the best standards of care available.
This will be a developmental post, progressing to band 6 after 12 months, subject to evidence of competency, extending to 24 months to achieve.
The candidate must have a full and valid driving licence and access to a vehicle for work purposes due to the community element of this role.
Student nurses waiting for the registration will also be considered.
Main Duties of the Job
1. Defined caseload that reflects the priorities of the local population and carry responsibility for the assessment of care needs; under the supervision of the CPN Clinical Lead.
2. Participate in the multi-disciplinary assessment of individuals with mental health problems living in the community.
3. Ensure that a risk assessment is completed for service users.
4. Be aware of the potential for vulnerability to abuse amongst the service users/families, and use the local policy to inform appropriate personnel in line with safeguarding vulnerable adults and children and SABP Policies.
5. Ensure that a carer's assessment is offered to carers of service users.
6. Utilise the full range of mental health nursing skills to participate in the assessment, care, and support of service users.
7. Participate in the provision of community mental health nursing advice and consultation to colleagues, community groups, and organisations.
8. Promote the recovery model to the people who use the service and their carers.
9. Act in a manner to respect the customs, individuality, values, sexuality, and spiritual beliefs, actively supporting the individual to fulfil these.
