An exciting opportunity has arisen to join a busy and passionate CMHT within the centre of Swansea as a CPN. The role will involve working as part of a multidisciplinary team to undertake the role of Care Coordinator, delivering high-quality community mental health services under the framework of the Mental Health Measure (2010) Wales, while maintaining a Recovery ethos at the forefront of nursing care.
Main duties of the job
Assess, implement, and evaluate care and treatment plans for each individual on the caseload, in collaboration with the service user and carers. Work autonomously, but under the indirect supervision of a Band 7 registered nurse or team manager, in conjunction with other health colleagues to support those under the care of EIP.

Detailed job description and main responsibilities
The successful candidate will work autonomously and as part of a team, conducting mental health assessments and serving as a point of contact for existing service users in the absence of their care coordinator. The role may also involve facilitating clinics within the CMHT, including physical health checks and medication administration according to local policy.

Qualifications
* Registered Mental Health Nurse
* Evidence of post-registration study, e.g., postgraduate diploma or equivalent registration experience
Knowledge
* Demonstrate clinical knowledge and understanding of current legislation such as the Mental Health Act 1983, Mental Health (Wales) Measure 2010, Mental Capacity Act, and Deprivation of Liberty safeguards
* Good knowledge of current care and treatment practices for adults with mental health needs
* Understanding of the Clinical Governance agenda
Experience
* Experience working within multidisciplinary and multi-agency teams
* Experience in risk assessment and management
* Experience at Band 5 or above in a related clinical environment
* Leadership experience within a team
Skills
* Excellent communication and interpersonal skills
* Ability to travel between sites and patient homes efficiently
